What format is the resume? JIS or MHLW?
It follows the common Japanese rirekisho layout close to the JIS standard and the format example published by the Ministry of Health, Labour and Welfare (MHLW) from 2021: name and furigana, date of birth, address and contact, education/work history, licenses, reasons for applying and a personal-requests box. Does it show the Japanese era date and my age?
Yes. Entering your date of birth renders it in the Japanese era calendar (Showa, Heisei, Reiwa, etc.) and shows your current age next to it. The date written is also shown in the era calendar.
